Systematic Review of the Studies Examining Costs and Cost Effectiveness of Radical Prostatectomy

Abstract

In the study, a systematic review of published scientific articles on the cost effectiveness and cost comparison of open radical prostatectomy, laparoscopic radical prostatectomy, and robotic radical prostatectomy methods used in prostate cancer surgery in the international literature was conducted between 2001-2017. The PRISMA method was followed in the preparation of the systematic review study using secondary data. International publications in PubMed, Web of Science, Science Direct, and National Institute for Health Research - Economic Evaluation databases were searched in line with the specified screening criteria. The results of the screening were included in the study of 27 publications. The robotic surgery is determined as more advantageous with respect to variables as publications received for study; the day of hospitalization at 10, complication rate at 5, re-admission at 3, and need for blood transfusion at 5. It has been determined that the cost of robotic radical prostatectomy surgery in 20 of the scientific publications included in the study is higher than the cost of laparoscopic radical prostatectomy and open radical prostatectomy. While there were no significant differences in the QALY values obtained between two prostatectomy methods in the two publication in study; two publications reveal that the amount of QALY gained by robotic radical prostatectomy surgery is higher. Evaluation of the robotic radical prostatectomy method, which requires large investments, taking into account the effects on the quality of life, may provide scientificness for taking investment decisions.
